for the detection of human immunodeficiency virus type 1 (HIV-1) in plasma.
METHODS:
The study was conducted in a randomized, double-blind, placebo-controlled, parallel-group, crossover design. The primary outcome was the percentage of plasma HIV-infected subjects who were seronegative for HIV. Secondary outcomes included the number of subjects with HIV infection and the time to HIV seroconversion.
,,, and were used to assess the effect of the treatment on the HIV status of HIV positive subjects. A total of 5,890 subjects were enrolled in the study. Plasma samples were collected from subjects at baseline and at the end of treatment. Serum HIV RNA was isolated from plasma samples by using a modified version of a method described previously (21). The HIV virus was purified by a reverse transcriptase method (22). Sera were stored at −80°C until analysis. HIV DNA was amplified by the method of Sanger (23).
, and, were analyzed for viral DNA using the Viral DNA Amplification Kit (VDA) (24). A positive result was considered to indicate that the subject was seropositive for at least one of two HIV types. For each subject, the mean number and percentage (percentage of sera positive for each type) of viral RNA isolated was calculated. In addition, a positive HIV test result for any of three HIV subtypes was used as a proxy for serostatus. All subjects had to be sero-positive for all three subtype HIVs. To determine the proportion of patients who had serologic evidence of infection with at most one HIV type, we used the ratio of positive serological results for subtyping HIV to the total number (positive serology) for that subtyping HIV (25). For the analysis of time, HIV was defined as the presence of at least one positive test for a sub-type of virus. We used a logistic regression model to estimate the change in HIV prevalence from baseline to follow-up. Statistical analysis was performed using SAS version 9.2 (SAS Institute Inc, Cary, NC).
