Abstract
True tinnitus is a phantom auditory perception arising from a source or trigger in the cochlea, brainstem, or at higher centers and has no detectable acoustic generator. The most accepted is the famous neurophysiologic model of Jastreboff, which stresses that tinnitus, is a subcortical perception and results from the processing of weak neural activity in the periphery. The aim of this study is to determine the role of Vitamin B12 in treatment of chronic tinnitus.
In this randomized, double-blind pilot study, total 40 patients were enrolled, of which 20 in Group A (cases) received intramuscular therapy of 1 ml Vitamin B12 (2500 mcg) weekly for a period of 6 weeks and Group B (20) patients received placebo isotonic saline 01 ml intramuscular. Of the patients with tinnitus, 63 had low B12 vitamin levels, and 37 had normal B12 vitamin levels. No significant change was observed in tinnitus severity after vitamin B12 therapy. Eight of these patients reported some relief in tinnitus on the VAS, but the rate of improvement was not significant (p > 0.05).
In the tinnitus patients with low B12 vitamin levels, audiometric tests conducted after B12 vitamin treatment revealed a significant improvement in hearing levels only at the 250 Hz frequency. Conclusion: B12 replacement treatment was not effective in these patients with tinnitus.

B12 Tinnitus Pilot Study
Participants were between the ages of 18 and 60 who were diagnosed with chronic subjective tinnitus. This means participants had frequent episodes of severe tinnitus lasting more than six months.
Group A received intramuscular therapy of 1 ml vitamin B12 (2500 mcg) weekly for the duration of the study. Group B received a placebo injection of 1 ml of saline. Before treatment started, participants had their B12 levels measured, completed a pure tone hearing test and filled out a self-report tinnitus severity index questionnaire.
These tests were completed again one month following their six-week treatment period.
