T vitamin B12 deficiency (VB12d): intramuscular (IM), oral, sublingual (SL), and intranasal vitamin B12 (VB12) preparations. The aim of the present study was to compare the efficacy of SL versus the standard IM administration of VB12 in restoring B12 levels.
Data were recorded for all patients older than 18 years of age who were prescribed VB12 during January 2014-December 2017. The mean ± SD difference between sVB12 levels before and after administration of VB12 supplements was significantly higher in the SL group vs. Im injection group (252 ± 223 vs. 218 ± 184 ng/L, p < 0.001). This is the largest study that documents therapy with SL preparations of VB12 sufficient and even superior to the IM route.

Abstract

Aims To compare the efficacy of sublingual and oral administration of 500 µg of cobalamin in subjects with cobalamin deficiency. Materials and results Thirty subjects with low serum concentrations of cobalamin participated in the study. Subjects were randomly allocated to receive one tablet daily of 500 µg cobalamin sublingually or orally, or two tablets daily of a vitamin B complex.
After 4 weeks, concentrations rose to 288 ± 74 pmol l−1, 286 ± 87 pmol l−1 and 293 ± 78 pmol l−1, respectively. Conclusion A dose of 500 µg of cobalamin given either sublingually or orally is effective in correcting cobalamin deficiency.

Side Effects

Cation by mouth, usually once daily or as directed.
Follow all directions on the product package. If you are taking a brand that contains vitamin C, take this medication by mouth with a full glass of water (8 ounces/240 milliliters) unless your doctor directs you otherwise. If you are taking chewable tablets, chew the tablet thoroughly before swallowing.
If you are taking extended-release capsules, swallow them whole. Doing so can release all of the drug at once, increasing the risk of side effects. Swallow the whole or split tablet without crushing or chewing.
Do not use a household spoon. Some liquid products need to be shaken before each dose. Some products that contain vitamin B12 need to be placed under the tongue and held there before swallowing.
Take this medication regularly in order to get the most benefit from it.
